History

Department was established in 1995 in old building in sir takhtsinhji hospital campus.before that department was part of district civil hospital and opd was running once a weekby private practitioner. after medical college establishied, all faculty positions were filled and daily opd &separate ward for department was established. with shifting of opds in new seven storiis opd & ward buildoing our dept, got opd with separate cubicals for all faculties and increased opd cases to 130-150 cases/day

NATIONAL PROGRAMME

VCTC Programme

Voluntary Counseling and testing for HIV infection is seen as a key entry point for a range of intervention in HIV prevention and care, therefore has NACO decided to set up a net work of VCT centers with an objective to make testing services available to those persons who wish to know their HIV status because of their indulgence in some risk behavior.

VCTC was set up in our institute in the year 2002. Counseling services are provided by trained counselors - 1 male and 1 female. There is a separate room for counseling of male and female counselors, with adequate waiting space for the patients. HIV testing is undertaken after proper pretest counseling and informed consent of the patient.

The confidentiality of the test result (whether positive or negative) is strictly maintained and recorded. VCTCs act as a link between HIV positive individuals and healthcare personnels and NGOs working in the field of HIV/AIDS in the district.

Services of VCTC are also utilized for counseling of STD patients. Various functions of VCTC may be summarized as below:

STD Clinic Activity and Reports

STD Clinic runs daily since Jan'2000. There is a separate consulting room for examining STD patients and privacy of the patients is maintained. There is a laboratory attached with the STD Clinic and facilities for diagnosis of STIs is available. Counseling center is attached with the STD Clinic and there is separate room for 1 male and 1 female counselor. Counselors educate the patients for safe sex practices, motivate patients for partner treatment, promote and supply condoms.

At first visit, drugs are supplied to the patients according to the guidelines prescribed in the syndromic approach to the management of STDs; and based on laboratory reports treatment is modified if required on subsequent visit.

Proper records of STD patients are maintained in the STD clinic.

Leprosy Clinic

• Leprosy Clinic (ANSUYA Leprosy Clinic and Counseling Center) is attached with Skin OPD, Sir T Hospital, Bhavnagar and runs every Friday Morning. Leprosy patients are seen daily in the OPD and are directed to come for follow up examination every Friday. Leprosy Supervisor assists the Doctor in smooth functioning of Leprosy Clinic. He takes smears of the patients, maintains proper records of the patients attending Leprosy Clinic and dispenses drugs to the patients.
• Patients with Lepra reactions and warranting hospitalization are admitted in the Ward treated accordingly. Patients with deformity / other medical illnesses are referred to concerned specialties for further management.

HIV Sentinel Surveilance

HIV Sentinel Surveillance means carrying out cross sectional studies (prevalence surveys) of HIV prevalence rates at regular intervals among selected groups in the population known as 'sentinel group'. HIV Sentinel Surveillance helps to monitor trends in HIV infection over time by group and by place.

The program is being implemented as per the guidelines prescribed by National AIDS Control Organization (NACO). Gujarat States AIDS Control Society (GSACS) selected Bhavnagar as a HIV sentinel surveillance site for STD patients from the year 2001.

Target Population: Patients attending STD Clinic attached with Skin OPD and Ob.Gy.OPD.

Sampling : Consecutive Patients with sexually transmitted disease attending STD clinic.

Sample Size : 250 patients or more over 12 weeks time (150 from Skin OPD and 100 from Ob.Gy. OPD) as per the guidelines prescribed by National AIDS Control Organization (NACO). Gujarat States AIDS Control Society (GSACS)

Frequency of Survey : Once a year - 12 weeks period in the months of July, August and September.

Testing Method : Unlinked Anonymous testing method is used. In STD Clinic, Blood is collected for VDRL, a part of it is separated and sent to the Microbiology Lab., V. S. Hospital, Ahmedabad for HIV testing after removing all personal identifiers namely, name, address, etc.

HIV Testing Strategy : The serum is first tested with one ELISA or Rapid or Simple Assay with higher sensitivity. Any serum found reactive on the first assay is retested with a second ELISA or Rapid or Simple Assay with higher specificity based on a different antigen preparation and/or different test principles.

Serum that is reactive on first test but non-reactive on second test is also considered antibody negative.

Analysis : Data collected at HIV sentinel site is reported and analyzed separately for each group. Primary data includes the number of sera examined at a site and the proportion of positive sera.

Interpretation of Data : The data is interpreted to assess the trend of HIV prevalence over time in the target sample group (patients with sexually transmitted diseases attending STD clinic).

Examination Pattern

Degree : M.D.(Dermatology, Venereology & Leprosy)

University Examination consists mainly of three parts :

1. Theory Examination
2. Practical Examination
3. Thesis Assessment

Theory Examination : 400 marks

Theory Examination will consist of 4 papers at the end of 6 academic terms. An academic term shall mean six months training period. The Chairman will take suggestions from remaining other examiners (internal & external) and draw a balanced paper. There will be four papers of three hours each with 100 marks in each paper-a total of 400 marks.

The paper will be drawn as follows :

Practical Examination : 600 marks

Practical examination shall consist of (a) Clinical (b) Oral examination & (c) Pathological Skills of 600 marks,

(a) Clinical examination for the subject in clinical sciences should be aimed at eliciting the Knowledge and Competence of the candidate for undertaking independent work as a specialist/teacher.

Clinical examination : 450 marks.

1. Long case : 200 marks.

All the two pairs of examiners shall simultaneously assess the candidate on long case. The candidate will be given 50 minutes while the assessment by examiners will be completed in 30 minutes; each pair of examiners will give marks out of 100.

2. Short cases : 150 marks.

Two pairs of examiners will assess 2 short cases, one by each pair, each giving marks out of 75.Each candidate will be given 20 minutes for examining each short case while a pair of examiners will assess the candidate for 15 minutes.

3. Spot Cases : 100 marks.

There will be 10 cases each will be carrying 10 marks. 10 minutes will be given for each case.

Oral examination : 100 marks.

There will be two tables each of 50 marks, each manned by a pair of examiners,will run simultaneously. It shall aim at eliciting the candidate's knowledge and competence about the subject, investigation procedures, therapeutic technique and other aspects of the specialty.

Pathological Skills : 50 marks.

There will be 5 histopathological slides each will be carrying 10 marks.

Dissertation Examination :

Every candidate shall carry out work on an assigned research project under the Guidance of a recognized Postgraduate Teacher, the results of which shall be written up and submitted in the form of a dissertation.

Work of writing the dissertation is aimed at contributing to the development of a spirit of inquiry, besides exposing the candidate to the techniques of research, critical analysis, acquaintance with the latest advances in the literature. Dissertation has to be submitted well in advance of Theory and Practical Examination i.e. at least six months before the examination.

The dissertation will be assessed by all the four examiners and its acceptance by majority of examiners shall be a precondition for the candidate to appear for the examination.

Passing Schedule :

Minimum passing marks for theory and practical is 50% independently and in of failure under any heading, the candidate will have to appear for whole exam after six months

Minimum 50% of examiners should be external. i.e. from other university .Of the externals at least one examiner should be from other state.
